Consistant use of namespace

Some of our templates force the namespace to be default. some use whatever the cli is set to use. We need to be consistent everywhere.

I suggest we specify it as a var in kolla-kubernetes.yml so it can be easily overridden and then specify it based on that var in all the templates.

That way, it will be much easier to run multiple kolla-kubernetes based control planes in the same k8s cluster and not accidentally cross the streams.
